Overview

We are seeking a dedicated and compassionate Health Home Registered Nurse Case Manager to join our team. This role is essential in providing comprehensive care management for patients, ensuring they receive the necessary support throughout their healthcare journey. The ideal candidate will possess a strong clinical background and be skilled in coordinating care across various settings, including inpatient, outpatient, and post-acute environments.

Duties

Conduct thorough patient assessments to develop individualized care plans. Coordinate and manage patient care across multiple disciplines, ensuring continuity of care. Collaborate with healthcare providers to facilitate discharge planning and follow-up care. Monitor patient progress and adjust care plans as needed based on clinical evaluations. Provide education to patients and families regarding health conditions, treatment options, and self-management strategies. Maintain accurate medical records in compliance with HIPAA regulations. Utilize critical thinking skills to evaluate diagnostic information and make informed decisions regarding patient care. Engage in utilization management activities to optimize resource use while ensuring quality patient outcomes. Support patients with complex needs including geriatrics, behavioral health, and infectious disease care. Participate in case management meetings to discuss patient status and share insights with the healthcare team.

Experience

Candidates should possess the following qualifications:

Critical care experience in settings such as ICU, emergency room, or Level I/II trauma centers. Familiarity with various medical specialties including pediatrics, geriatrics, hospital medicine, outpatient clinics, and medical-surgical nursing. Knowledge of medical terminology, physiology, and patient service best practices. Experience with discharge planning, intake processes, and case management principles. Strong understanding of acute care protocols and post-acute care transitions. Proficiency in conducting diagnostic evaluations and managing medical records effectively. Pay: $86,501.33 - $104,173.64 per year
