e are seeking to hire a Home Health Registered Nurse Case Manager who will play a vital role in coordinating comprehensive, patient-centered care for individuals in a home setting.

Pay Rate: Range $2200 to $2240 Weekly Gross pay based on a 40-hour work week

(Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement:

$0.49/mile for distance covered between patient home appointments

Shift timings:

8 Hours; Days, 8a - 5p with some on-call



Schedule:

40 hours/week guaranteed, 5 shifts per week, Mon-Fri, No weekend or on-call

Position Requirements:



Registered Nurse: 2 years - Required Recent/Current Home Health Nursing experience: 1 year - Required OASIS experience - Required Home Care Home Base (HCHB) charting experience - Required Must have valid Driver's License and Personal Auto Insurance

Education, License and Certification Requirements:



ASN or an equivalent - Required BSN or an equivalent - Highly preferred RN License for the state applying or Compact if accepted - Required BLS Certification from AHA or American Red Cross - Required

Provide and coordinate care for assigned patients in comfort of their homes. Develop and implement nursing care plans including Start of Care and admissions as assigned. Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy. Empower patients and families to reach healthcare goals by educating them on disease management. Provide Home health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure physicians' orders are legible if taken verbally and transcribed appropriately per policy and procedure. Ensure patients' status is assessed on an ongoing basis and pertinent information is gathered and documented. Notify physicians of significant changes in status. Attend IDT meeting as required Complete patient visits as assigned to meet the minimum required weekly productivity points (30) Document and complete charting for all patient visits within 24 hours. Territories to cover: Green Bay and outlying areas
