Summary

JOB SUMMARY:

The prospective Care Coordinator provides a wide range of support for Health Home Staff, medical staff, and other departments. Routinely collects urine specimens and breathalyzers for monitoring of illicit drug use, alcohol use, and program compliance. Clinic is open from 7:00 AM - 5:30 PM; Candidate would work an 8 hour shift to cover requirements through the day which will require different start/end times based on patient load.
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Demonstrates awareness of federal confidentiality requirements and maintains confidentiality of patient protected health information.
  • Ensures appropriate maintenance of departmental equipment and office manuals.
  • Prepares a variety of administrative reports, statements and rosters; gathers data and other necessary information. (Data Management and reporting).
  • Establishes, maintains, and revises record keeping and filing systems; classifies, sorts and files correspondence, records and other documents.
  • Registering patients and Tracking Health Home referrals.
  • Scheduling Health Home staff and enrollees.
  • Assist with chart audits for compliance.
  • Reminds enrollees regarding keeping appointments, filling prescriptions, etc.
  • Request and sends medical records for care coordination.
  • Answer telephone calls, record and deliver messages.
  • Process incoming/outgoing mail and faxes as needed.
  • Assist managers with typing, filing and computer related skills as needed.
  • Assist with daily clerical operations as needed.
  • Perform routine and non-routine activities involved in the preparation of patient specimens for processing.
  • Assure specimens and labeling are correct.
  • Maintain the integrity of the specimen and patient identification.
  • Generate weekly urinalysis reports.
  • Data Entry specimen on template and Methasoft.
  • Demonstrated ability to handle broader range of specimens with indirect supervision (i.e. oral swab). Generate daily and weekly urinalysis reports.
  • Conduct Breathalyzer on patients.
  • Complete all associated paperwork and maintain stock (i.e. bottles, swabs, etc.).
  • Identify problems that may adversely affect test performance or reporting of test results and either correct the problems or immediately notify the supervisor/manager.
  • Ensure all safety, quality control and quality assurance standards are met.
  • Follow CLIA, HIPAA, OSHA, and safety guidelines as described in their respective guidelines.
  • Maintain a clean and well-organized work area.
  • Completes training and competency checklists as appropriate
  • Other duties, as assigned.
REQUIREMENTS:
  • Bachelor\xe2\x80\x99s Degree in Health-Related field with two years of Health Care experience preferred.
  • Experience working with patients with history of addiction and/or mental health disorders preferred.
  • Working knowledge of computer software programs, Microsoft Office Suite, and knowledge of operating standard office equipment.
  • Must have excellent verbal and written communication skills.
  • Good research skills and attention to detail.
  • Accurate performance with alpha/numeric lists.
  • Ability to interact tactfully and courteously with public.
  • Ability to maintain confidentiality of patient protected health information.
  • Current/active CPR certification from the American Heart Association.
