Resident services coordinator position details:

We are hiring for a Part Time, 2 days/week position.

Resident services coordinator job duties include:

Coordinate the resident services program for the entire community at the Canterbury Woods Gates Circle location in Buffalo, NY. Participate in admissions, transfers, discharge planning, and development and implementation of service plans and resident assessments. Maintain and appropriately follow through on resident Accident and Incident reports. Input and maintain resident emergency contact sheets. Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures. Collect and coordinate all resident insurance information and policies, including annual confirmation of Medicare eligibility. Assist in providing solutions for social and practical environmental problems including discharge planning and referrals to other community agencies when required.

Resident services coordinator qualifications:

Must possess a bachelor's degree in social work. * Must have experience working in a long-term care or senior services setting.
