Functional Medicine is an innovative approach to health and wellness. The Functional Medicine Nurse supports the functional medical and obesity clinic providers and assists with providing care and tools designed to optimize patients' health.
ESSENTIAL FUNCTIONS

Primary Functions:
Responds to calls and messages from patients, triaging and prioritizing requests for clinical advice, care plans, prescription refills, test results, prior authorizations, forms completion, and coordinating care with providers and other nursing team members
Refills medications as authorized per protocol and as ordered by the provider(s)and obtains insurance authorizations as needed
Reviews and prepares charts for upcoming appointments including retrieval of pending test results, consult reports, and external documents
Conducts patient assessments, coordinates development of weight loss plans, monitors vital signs and progress and provides ongoing education support about nutrition, exercise, and healthy lifestyle as team member of obesity clinic
Documents patient care, medication management, patient education and communications, and care coordination in electronic health record (EHR) utilizing standard documentation guidelines
Demonstrates knowledge of nursing principles, practices, and procedures (includes vitals, immunizations, medications, in-office lab testing and procedures, and sterilization methods) and regulations related to patient safety, infection control, emergency operations and confidentiality
Assists with clean up in the team room at end of day
Participates in clinic process improvement activities and team meetings to ensure efficient operations
Reliable attendance and punctuality
Performing other duties as assigned
POSITION QUALIFICATIONS
Certificates & Licenses:
Kansas licensed RN or LPN
IV certification is preferred
Education:
Associate's degree (two-year college or technical school)
Experience:
Minimum two (2) years nursing experience required
EHR experience preferred (we use eClinicalWorks)
Microsoft Office experience a plus
