What You'll Do
As a Registered Nurse Case Manager, you'll do more than carry out clinical tasks--you'll support healing, protect dignity, and help patients live as fully as possible in the comfort of home.
You will:
Lead with care and competence
- Assess, plan, supervise, and guide patient care with expertise and compassion.
Provide personalized care
- Complete in-home assessments and develop individualized treatment plans.
Stay accurate and compliant
- Document thoroughly, follow all state and agency guidelines, and use up-to-date technology and best practices.
Collaborate with purpose
- Work closely with the interdisciplinary team to ensure exceptional outcomes.
Make meaningful moments
- Sometimes your presence will be what brings comfort, hope, and a smile.
What We're Looking For
Current
Utah RN license
Nursing experience preferred (Home Health or Hospice experience is a plus)
Strong communication skills--both written and verbal
High level of professionalism, respect for privacy, and strict confidentiality
Valid
Utah driver's license
and proof of auto insurance
Current
CPR certification
