Job Description

Our chosen candidate will support our patients and families in the Charlottesville and Augusta County areas.

Job responsibilities can include but are not limited to:

Administer skilled nursing for all ages in their place of residence. Conduct patient admissions. Conduct routine patient visits. Coordinate care within a hospice interdisciplinary team to assure patient/family needs are met. Communicate with attending physicians, hospice physicians, and other physicians involved in the patient's care. Demonstrates accountability, flexibility, respect, and integrity. Submits all required paperwork and documentation within 48 hours.

Qualifications:

Current Registered Nurse license in the state of Virginia. . One (1) years experience in a hospice setting heavily preferred (but not required) as a RN Case Manager or similar role. * Must have a current Texas Driver's license and dependable transportation.
