Purpose:

To provide case management activities with a range of individuals and families who are economically marginalized and/or with limited English proficiency through a holistic approach that promotes stability and well-being.

Essential Functions:

Manage a caseload of clients.

Provide concrete services such as completing eligibility forms, resource coordination, securing government assistance, etc.

Conduct client assessments, identify goals, establish care plans and complete quarterly reviews.

Network with agency and community service providers.

Stay current on general community resources and local/federal entitlement programs and assist clients in obtaining them.

Track, monitor, case note and report client progress.

Collect demographic and outcome data on all clients and complete reports according to schedule.

Provide administrative assistance to clients on issues relating to completing insurance forms, securing government assistance, etc., including paperwork and in-person advocacy as appropriate.

Serve as Information & Referral (I&R) services with agency clients.

Secondary Duties:

Maintain accurate, complete and timely client records and associated notes in the mandated systems according to agency and HIPAA policies.

Attend staff, supervisory, team and impact area meetings.

Travel Requirement:

Considerable local travel (Louisville Metro area) required.

Pre-Employment Requirements:

Satisfactory completion of a pre-employment background and drivers' record check. Negative TB test and drug screen required.

Education, Training and Experience Requirements:

Bachelor's in human services or related field, preferred. Bi-lingual in Arabic or Spanish, preferred.

Performance Requirements:

Responsible for meeting agency, grant and other funder outcomes in a timely manner.

Attend all agency and department meetings.

Meet reporting and departmental deadlines.

Maintain up-to-date client case records and outcomes measurement as mandated by internal policies and procedures, and by those with contracted organizations.

Handle all Protected Health Information (PHI) and Personally Identifiable Information (PII) confidentially and follow agency mandates procedures regarding the security of client/agency information.

Conduct self in a professional and respectful manner with coworkers and community members.

Knowledge, Skills and Abilities:

Ability to develop constructive, professional and cooperative working relationships with others and maintain those relationships over time.

Effectively utilize general and program-specific computer programs (e.g., Microsoft Office Suite, Penelope, etc.)

Effectively operate typical office equipment (telephone, computer, fax machine, copier, mail machine, etc.)

Thorough knowledge of community resources to assist participants with needs of food, shelter, transportation, childcare, financial assistance, etc.

Sensitivity to different cultural, socio-economic and age groups.

Physical Requirements and Environmental Conditions:

Tasks involve sitting, standing, walking, kneeling, bending, crouching, reaching and performing other physical requirements commensurate with demands of the position, specifically conducting home visits.

Sedentary work which involves sitting most of the time.

Must be able to speak (expressing or exchanging ideas by means of spoken words)

Must be able to hear (hearing aids acceptable)

Close visual acuity to perform activities such as viewing a computer terminal, excessive reading, and preparing and analyzing data and figures (corrective devices acceptable).

The worker is not substantially exposed to adverse environmental conditions (typical office work).

Pay: $42,436.00 - $47,740.00 per year
