Float Direct Support Professional- Joe's Place - $19.28/Hr

FT 32 Hr/wk
Provides 24/7 support to program participants in a residential program. Eligible participants fall under the jurisdiction of the Psychiatry Security Review board (PSRB). Responsibilities include ensuring participants are following a conditional release plan under the jurisdiction of the PSRB, supporting activities according to the program participant's IRPs, assisting program participants in performing daily living skills and personal care needs, providing emotional support to participants, monitoring participants physical and mental health, and reporting relevant information to the Director. Documentation of services in the Electronic Medical Records System (EMRS). CNA/HHA/PCA/PCT preferred.
Implements designated goal activities in coordination with the Service Coordinator or Director/Coordinator
Provide crises intervention supports
Conducts light housekeeping, disinfecting, yard maintenance and snow removal on sidewalks and emergency exits
Provides emotional support to program participants
Monitors program participant's physical and mental health and report relevant information to the Service coordinator or Director/Coordinator
Documents interaction in the program's communication log
Monitors and reports program participant's compliance to their conditional release order when applicable
Provides coverage as needed at any of the Agency sites per Agency need
Engages in continuous concerted efforts to access, obtain, respond to pertinent Agency information/communication, and disseminate as appropriate
Provides working conditions that maximize safety for staff and program participants
Maintain CPR/FA certification
Other responsibilities of Reliance Health Staff, such as participation in Agency events, adhering to Agency policies and procedures, and training requirements, etc.
Performs duties while demonstrating Agency core competencies and values
Education and Experience:
High School Diploma or G.E.D
Valid CT Driver's License, vehicle registration and insurance
Good Driving record
Bi-lingual preferred
