We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult patients. This role involves managing acute and chronic conditions, performing diagnostic evaluations, and collaborating with multidisciplinary teams to deliver exceptional patient-centered care. Experience with EMR/EHR systems is essential; eClinicalWorks is preferred. The Family Nurse Practitioner will also be involved in health coaching, ER follow ups and utilizing advanced clinical skills in various settings including outpatient clinics and telehealth platforms.
Responsibilities
Conduct thorough physical examinations and patient assessments across a broad age range, including infants, children, adults, and seniors.
Diagnose and manage acute and chronic illnesses utilizing clinical research data and evidence-based practices.
Utilize EMR/EHR systems like eClinicalWorks for accurate medical documentation and coding (CPT, ICD-9/10).
Provide specialized care in areas such as geriatrics (memory care/dementia), hospice & palliative medicine, infectious disease care, behavioral health, occupational health, and post-acute care.
Engage in telehealth consultations leveraging telehealth experience to expand access to care.
Maintain compliance with HIPAA standards and infection control protocols to ensure patient safety.
Requirements
Valid Family Nurse Practitioner certification/license with current state registration.
Demonstrated experience with EMR/EHR systems such as Epic, eClinicalWorks or Athenahealth is required; familiarity with eClinicalWorks is preferred.
Extensive clinical experience in family practice or outpatient setting is preferred.
Knowledge of physiology and anatomy relevant to pediatric , adult and geriatric populations.
Ability to work independently while collaborating effectively within multidisciplinary teams.
Strong understanding of medical terminology, diagnostic evaluation techniques, medication administration protocols--including psychopharmacology--and medical records management.
