Job Summary

We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including outpatient clinics, assisted living facilities, and urgent care environments. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.

Duties

Conduct thorough physical examinations and patient assessments to diagnose and manage acute and chronic conditions. Develop and implement individualized care plans based on patient needs and preferences in coordination with Hospice Medical Director Utilize EMR/EHR system Brightree/Matrix (ipad) for accurate medical documentation and coding (ICD-9/ICD-10). Collaborate with interdisciplinary teams to coordinate care for patients in home care, nursing homes, or hospice settings.

Experience

Candidates should possess a strong background in nursing with experience in the following areas considered advantageous:

Internal medicine or family practice Adult and geriatrics Assisted living or nursing home environments Knowledge of behavioral health principles Familiarity with pain management techniques including acute pain management and palliative care Proficiency in medical terminology and documentation practices Understanding of workers' compensation law and utilization review processes

Job Type: Contract

Pay: $51.06 - $61.49 per hour

Expected hours: 1 - 6 per week
