Job Overview

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our healthcare team. This role involves providing comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult medicine. The ideal candidate will have extensive experience with EMR/EHR systems such as Epic, Athenahealth, and eClinicalWorks, and possess a broad skill set that includes acute and chronic disease management, telehealth, and specialized procedures. The Family Nurse Practitioner will collaborate with physicians, specialists, and interdisciplinary teams to deliver exceptional patient-centered care in various settings such as outpatient clinics, inpatient units, and post-acute care facilities.

Duties

Conduct thorough physical examinations and patient assessments to diagnose health conditions. Manage acute and chronic illnesses including pain management, infectious diseases, geriatrics, and behavioral health issues. Perform procedures such as suturing, IV insertion, catheterization, phlebotomy, venipuncture, sterile processing, and sterile technique adherence. Administer injections, vaccinations, dermal fillers, botulinum toxin treatments, and esthetic laser treatments. Provide specialized care in areas such as hospice & palliative medicine, Alzheimer's and dementia care, memory care, neonatal ventilator management, gastrostomy tube feeding, spinal taps, and airway management. Coordinate discharge planning and utilization review for inpatient and outpatient services. Engage in telehealth consultations utilizing systems like Cerner and eClinicalWorks to expand access to care. Assist with medical documentation including CPT coding, ICD-9/10 coding, ICD coding for diagnostic evaluation. Educate patients on health coaching topics such as nutrition, family planning, medication administration, and disease prevention. Ensure compliance with HIPAA regulations and infection control protocols across all clinical activities. Collaborate with multidisciplinary teams in settings like Level I Trauma Centers or Level II trauma centers for trauma-related care or ICU experience. Participate in case management activities for patients with complex needs including post-acute care or working with people with disabilities or developmental disabilities.

Experience

Proven clinical experience in primary care settings including outpatient clinics or hospital medicine. Experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, eClinicalWorks. Background in acute care environments such as ICU or emergency room experience is highly desirable. Familiarity with specialized procedures including gastrostomy tube placement, spinal taps (lumbar punctures), cardiac catheterization support or ventilator management preferred. Experience working with diverse populations including pediatrics (neonatal to toddler), geriatrics (memory care or Alzheimer's/dementia), hospice & palliative medicine or behavioral health is advantageous. Knowledge of Workers' Compensation law and utilization management practices is a plus. Ability to perform diagnostic evaluations utilizing laboratory experience or medical imaging techniques; proficiency in medical terminology essential. Pay: $42.00 - $49.00 per hour
