EQIP Practice Transformation Grant Project Manager

The EQIP Practice Transformation Grant Project Manager will provide consistent oversight of MedChi sub-vendors, timely reporting, and transparent documentation of services rendered to EQIP entities throughout the grant cycle. The project manager will act as a liaison between the EQIP entities, MedChi sub-vendors, and CRISP to monitor progress, identify risks, and uphold compliance with EQIP Practice Transformation (PTG) program requirements.
RESPONSIBILITIES AND COMPETENCIES INCLUDE THE FOLLOWING:
Responsibilities include:
Initiate and maintain communication with all MedChi PTG EQIP entities.
Oversee and document all activities provided to EQIP entities by vendors.
Analyze trends, successes, and challenges across entities.
Manage all vendor relations including vendor contracts.
Monitor vendor engagement to confirm alignment with EQIP goals and PTG scope.
Ensure vendors maintain evidence of services rendered.
Document and escalate issues such as lack of responsiveness or missed milestones.
Complete a monthly report for each EQIP entity using the standardized EQIP PTG template and meet with CRISP to review.
Complete quarterly check-ins and year-end evaluations of entity performance.

QUALIFICATIONS
Project management experience. Additional experience with practice management, physician offices, value-based care and transformation work preferred but not required.
COMPENSATION
This is a one-year contract position. Depending on scope of contract, compensation is $50,000 - $90,000.
