Position Summary:
Under Terros' Health's policies and professional requirements, participates as a member of an interdisciplinary clinical team working with individuals diagnosed with a General Mental Health and Substance Abuse diagnoses. Completes annual assessments, crisis treatment plans, individualized service plans, and outreach assistance. Communicates and documents client progress and all outreach efforts in a professional, efficient, and timely manner. Uses brief clinical interventions, motivational Interviewing, and supportive listening skills to engage and support clients receiving services. Works under the direct supervision of the Integrated Care Site Director or Clinical Site Manager.
Duties Include:
Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with clients receiving behavioral health services.
Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate. Coordinates care with a client's family, behavioral and general medical providers and other involved supports including educational, social, judicial, and other State agencies.
Participates in clinical staffing's, case conferences, or other meetings, with or without the client or his/her family or other natural support participation.
Provides excellent customer service to clients, internal and external provider agencies, State entities, or other stakeholders (e.g. probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.) as needed.
Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty services.
Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the client, input from the person and relevant others, and consultation with the clinical team, resulting in modification to the service plan as necessary.

Education
:
+ High School diploma/GED equivalency and 3 years of behavioral health experience, or
+ Associates degree in Medical or Behavioral Health field; or
+ A bachelor's degree in the field of behavioral health and any behavioral health experience, or
+ A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience. Basic understanding of the DSM-V preferred.
High level of competency in Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
Excellent communication skills, evidenced by a non-judgmental approach and effective listening.
Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
Valid AZ Driver License. Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy
Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
Must pass a TB Test.
