Job Description

Key Responsibilities




Outpatient Clinic & Program Development (Primary Focus)



Serve as the

clinical lead

for the AFib Center of Excellence program. Coordinate multidisciplinary care for patients with both paroxysmal and persistent atrial fibrillation. Manage anti-arrhythmic drug therapy initiation, titration, and ongoing monitoring. Provide comprehensive education to patients and families on AFib, stroke prevention, rhythm/rate control strategies, and lifestyle modifications. Develop and maintain standardized care pathways and patient education materials. Collaborate with the electrophysiology physician to design protocols for

left atrial appendage (LAA) management strategies

including referrals, follow-up, and patient counseling.

Inpatient Support (Secondary Role)



Round on inpatients with AFib, post-ablation, and other arrhythmia-related conditions as needed. Assist in the transition of care from inpatient to outpatient AFib clinic, ensuring continuity and adherence to care plans.

Device Management (Adjunct Role)



Support evaluation and follow-up of patients with implanted cardiac devices (e.g., pacemakers, ICDs, loop recorders) as related to AFib detection and burden monitoring. Assist in interpreting device data to inform AFib management strategies.

Care Coordination & Quality Improvement



Foster collaboration between cardiology, primary care, pharmacy, and allied health teams to optimize patient outcomes. Participate in data collection, quality metrics tracking, and process improvement initiatives to maintain

center of excellence status

. Support the development of educational seminars, community outreach, and professional training programs on AFib care.

The successful candidate will be trained by our board certified Electrophysiologist and serve as a key member of the EP service care team, helping support the development of the A-Fib program at Tanner Health.

Development: Maintain and update level of skill for pertinent medical assistant duties. Participates in continuing education. Participates in the development of new programs (ex: Process Improvement, Quality Initiatives). Patient Care: Performs evaluations, provide treatment and counsel patients. Order and interpret test results and recommend treatment of patients. Administer therapeutic procedures. Prescribe therapy or medication with physician approval. Provide physicians with assistance during surgery or complicated medical procedures. Demonstrates specialty and population specific competencies. Demonstrates an ability to effectively communicate based on specific specialty and population. Performs other duties as assigned by the supervising physician. Quality: Maintains evidence based medical care and clinical charting. Participates with clinical tracking and feedback. Integrates with clinical and administrative teams to administer high quality care. Teamwork: Works well with others and functions as a team player. Offers ideas and solutions for issues that affect the team and work area. Participates in the training and skills development of new clinical staff. Assists as needed.

Licenses & Certifications



Georgia Nurse Practitioner License Georgia Licensed Practical Nurse or Multistate Nursing License Healthcare Provider (CPR)


Supervision



Regularly uses assistance and leads the work of one to four co-employees, may instruct new employees and correct work routines, checks results and may discuss with employee's superior.


Qualifications



Able to communicate effectively with medical and hospital staff and various populations in clinics and public screening events. ACLS recommended. Additional certification will be required for some specialties (i.e. ACLS, PALS) Current certification as a Nurse Practitioner in Georgia Graduate of a recognized program for Nurse Practitioner training (MSN). Must be capable of dealing tactfully and effectively with patients, family members, other employees and physicians Must maintain high degree of confidentiality. Must meet credentialing criteria. Must possess the cognitive ability to understand patient medical records, physician orders and medical orders to be communicated to patient, hospital and/or other physician. Must possess the sense or discipline to work in accordance with accepted clinical standards. Prior experience in a like specialty preferred.


Definitions



Nurse practitioner provides patient services through the performance of patient evaluation, assessment and treatment in the medical office setting as allowed by state licensure laws under the supervision of the medical director. The nurse practitioner works closely with the clinic staff, hospital departments and community agencies.

Effect Of Error



Probable errors not easily detected and may adversely affect external as well as internal relationships and may result in major expenditures for equipment, materials, or procedures detrimental to the patient's welfare or the organization's interest. Work is subject to general review only and requires considerable accuracy and responsibility. Continually works with reports, records, plans, and programs of a major functional area of the organization where integrity is required to safeguard the organization's position. Duties may involve the preparation of data on which the administration bases important decisions and are highly confidential.



Supervisory Responsibility



Supervises a small section of employees (up to 15), orients and trains employees, schedules, assigns, checks, and maintains flow of employees' work, carries out most personnel decisions, and initiates minor personnel actions.




Mental Demands



Work involves a variety of problems in a general field, some of which are complex. Involves some independent judgment to decide what to do to assemble facts, determine variations from standard procedures, or plan other action to be taken to meet general objectives.




Physical Effort



Moderate physical effort - Lifts, carries, or handles lightweight (1 to 25 lbs.) materials or equipment for about half of the day. Very occasional physical effort with medium weight objects (25- 60 lbs.). Office or laboratory work requires close visual effort and concentration more than half of day. Works in reaching or strained positions for less than half of day.



Working Conditions



Moderate - (About 50% of the day) Involved in exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes/occupational risk. Limited probability of coming into contact with blood borne pathogens, other potentially infectious diseases, or biomedical/bio-hazardous materials.
