Position Summary



The Early Supports and Services (ESS) Service Coordinator is responsible for the case management/service coordinator of Community Crossroads' ESS program, which serves children from birth to age three with developmental delays or risk factors. This role includes direct family engagement, coordination with vendor agencies, compliance with state and federal regulations, and participation in quality assurance efforts. The ESS Service Coordinator ensures families receive timely, effective, and supportive services in accordance with best practices and contractual requirements.

Responsibilities



Serve as a point of contact for families, alongside the ESS Program Coordinator, guiding them through intake, eligibility, service coordination, and transitions to preschool. Support tracking Individualized Family Support Plans (IFSPs), ensuring timely and goal-focused services. Help coordinate with families, providers, and school districts to support service delivery and smooth transitions. Monitor vendor services and documentation for quality, accuracy, and compliance with state and federal guidelines. Maintain accurate records and support data tracking for program performance. Address family or provider concerns, resolving issues collaboratively and efficiently. Provide referrals, promote family training opportunities, and ensure access to additional supports. Participate in agency initiatives, stay current on ESS regulations, and represent the organization at meetings and trainings, as needed.

Requirements



Bachelor's degree in Early Childhood Education, Special Education, Social Work, Human Services, or a related field; Minimum of 2-4 years of experience in early intervention, developmental services, or family support roles. Working knowledge of ESS systems, IDEA Part C, and NH He-M 510 regulations. Strong organizational and time management skills, with attention to detail and documentation standards. Excellent interpersonal and communication skills, with an ability to engage and support families, vendors, and agency staff effectively. Self-motivated and able to manage responsibilities independently in a fast-paced environment. Proficiency in electronic documentation systems, Microsoft Office, and data tracking tools.
