Job Description

The Case Manager/Discharge Planner, Employee Health Nurse, and Clinical Educator is a multifaceted nursing professional who coordinates patient care from admission to discharge, oversees employee health services, and develops and implements educational programs for clinical staff. This position ensures optimal patient outcomes, promotes a safe and healthy workforce, and fosters a culture of continuous learning and quality improvement. Reports to the Chief Nursing Officer (CNO).

Case Manager/Discharge Planning



Coordinate and facilitate the clinical care of patients to promote efficient resource utilization and patient outcomes. Collaborate with interdisciplinary teams to assess, plan, implement, and evaluate patient care plans. Develop and manage individualized discharge plans in coordination with physicians, nursing staff, patients, families, and community resources. Ensure compliance with regulatory requirements for discharge planning and case management. Track readmissions and assist with performance improvement initiatives related to care transitions.

Employee Health



Coordinate pre-employment and annual employee health screenings, immunizations, and tuberculosis testing. Monitor employee exposure incidents (e.g., needlestick injuries, infectious diseases), follow up appropriately, and maintain records per OSHA standards. Maintain employee health records in a confidential and organized manner. Ensure compliance with regulatory and accreditation standards related to employee health (e.g., OSHA, CDC, The Joint Commission).

Nurse Educating



Plan, develop, coordinate, and evaluate orientation programs for new clinical staff. Provide ongoing education to nursing and clinical staff based on identified learning needs, regulatory updates, and performance improvement data. Support staff competency validation and skills training programs. Assist in the development and revision of clinical policies, procedures, and protocols. Participate in mock surveys, drills, and emergency preparedness training.

JOB QUALIFICATIONS:



Education:


Associates in Nursing

Bachelor's degree in nursing preferred

Previous experience in discharge planning, clinical education, or employee health preferred.

License:


Current RN license in the State of Louisiana.

BLS/CPR certification is required

ACLS certification is required

CPI/MOAB certification is required

Job Type: Full-time

Pay: $60,864.95 - $73,299.73 per year
