Discharge Planner Registered Nurse
Job Description
We're looking for an experienced and passionate Discharge Planner (RN) to coordinate discharge planning with the resident/guest's attending provider and assist in contacting appropriate community resources. Responsibilities include:
Coordinates and manages discharge planning activities in compliance with all federal and state regulations.
Meets with resident/guest and/or family within 72 hours of admission to obtain resident/guest/family goals for discharge.
Collaborate with healthcare team, including providers, licensed nurses and social workers.
Facilitate resident/guest referrals to external facilities/providers that are needed for continuation of care.
Assist in arranging timely home health services and equipment for discharge.
Requirements:
Graduate of an accredited school of nursing. Current license as a Registered Nurse in the State of Hawaii required. 4 or more years' experience in restorative and gerontological nursing. Current and valid 2 Step TB clearance and health review (can be done in-house). First Aid and CPR preferred. (can be done in-house). Discharge planning experience, preferred.

Pay: $100,000.00 - $105,000.00 per year
