We are seeking an experienced and motivated

Director of Social Services

to join our leadership team. This role is ideal for someone who is passionate about advocating for residents, supporting families, and leading a dedicated social services department in a long-term care setting.

Responsibilities

Oversee and manage all functions of the Social Services Department.



Conduct psychosocial assessments and develop individualized care plans for residents.



Provide counseling and support to residents and families regarding adjustment to facility life, discharge planning, and access to community resources.



Collaborate with the interdisciplinary care team to ensure holistic, person-centered care.



Maintain compliance with federal, state, and facility regulations.



Document and maintain accurate resident records.



Supervise and guide social services staff to promote professional growth and quality outcomes.

Qualifications

Bachelor's or Master's degree in Social Work (MSW preferred).



Current Massachusetts Social Work license (LSW, LCSW, or LICSW).



Minimum 2 years of experience in a long-term care or healthcare setting.



Strong leadership, communication, and organizational skills.



Empathetic, resident-centered approach to care.
