We are seeking a compassionate and dedicated Direct Support Professional to provide high-quality care and assistance to individuals with developmental disabilities, aging-related conditions, or other health challenges. The ideal candidate will possess a strong commitment to promoting independence, safety, and well-being while fostering a respectful and supportive environment. This role involves working closely with residents or clients to support their daily needs, monitor health status, and facilitate community engagement. Responsibilities

Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary. Monitor and record vital signs, patient observations, and behavioral changes to ensure health stability and communicate findings to healthcare team members. Administer medications following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality. Prepare nutritious meals and assist with meal planning in accordance with dietary restrictions and individual preferences. Maintain a clean, safe, and organized environment by performing routine cleaning tasks and ensuring compliance with sanitation standards. Support residents in participating in community activities, social outings, and recreational programs to promote social integration and quality of life. Follow established care plans tailored to each individual's needs, including dementia care, Alzheimer's care, autism support, or hospice care when applicable. Document all care activities accurately and promptly while respecting residents' privacy rights under HIPAA guidelines.

Experience

Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated knowledge of patient monitoring techniques, medication administration, and vital signs measurement. Familiarity with specialized care such as dementia care, memory care, or hospice support is preferred. Experience assisting individuals with disabilities or developmental challenges using person-centered approaches. Knowledge of safety procedures including CPR and first aid certification is required; additional training in heavy lifting or the use of assistive devices like Hoyer lifts is advantageous. Strong background in caregiving roles involving long-term care or home & community care settings. Ability to work effectively within multidisciplinary teams including social workers, nurses, and other healthcare professionals while maintaining confidentiality under HIPAA regulations.
