We are seeking compassionate and dedicated Direct Support Professionals to provide high-quality care and support to individuals with developmental disabilities, aging needs, or other special care requirements. This role involves assisting residents with daily activities, ensuring their safety, and promoting independence within a supportive environment. The ideal candidate will have a strong commitment to person-centered care and possess relevant experience in home and community care settings.
Responsibilities
Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor and document residents' health status, vital signs, and behavioral changes in accordance with care plans
Administer medications and assist with medication management following HIPAA guidelines
Support residents during meal preparation, feeding, and nutrition management
Provide patient observation and ensure safety during activities both within the facility and in the community
Assist with mobility using equipment such as Hoyer lifts or other assistive devices
Maintain a clean and safe environment by performing light cleaning duties as needed
Implement individualized care plans tailored to each resident's needs, including dementia, Alzheimer's, autism, or other specialized care requirements
Support residents' participation in social activities and community outings to promote engagement and well-being
Follow all safety protocols including CPR and first aid procedures to respond effectively in emergencies
Skills
Proven experience in assisted living, long-term care, or home health environments; group home experience is a plus
Knowledge of patient monitoring, vital signs collection, and medication administration
Strong understanding of dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism
Ability to perform heavy lifting when necessary for resident transfers or mobility assistance
Excellent patient observation skills to detect changes in condition or behavior
Compassionate caregiver with excellent communication skills and patience
Ability to work independently as well as part of a team in a fast-paced environment
Knowledge of HIPAA regulations related to patient privacy and confidentiality
CPR and First Aid certification preferred; willingness to obtain if not already certified
