Position Summary
The Designated Coordinator will oversee service delivery and program operations for individuals supported by our agency. This person will ensure compliance with Minnesota Statutes 245D requirements, support person-centred planning, supervise direct support staff, monitor outcomes, and collaborate with case managers, guardians, and community supports.
Key Responsibilities
Conduct intake and admission activities with persons served and their teams.
Develop, implement, monitor, and update Individual Support Plans (e.g., CSSP, IAPP, Support Plan Addendum) in accordance with licensing regulations. homecareagencymn.com+2Holt Law+2
Supervise, train, evaluate Direct Support Professionals (DSPs) so service delivery aligns with the support plans and meets quality standards. members.arrm.org+1

Monitor service outcomes and staff competency; observe direct support staff as needed, and participate in program evaluation and improvement efforts. MN Revisor's Office+1
Coordinate with external stakeholders (county case managers, guardians, medical providers, behavioral health professionals) to ensure continuity and quality of supports. JobTarget
Support crisis intervention and safety planning, address behavioral/medical concerns, ensure compliance with safety, abuse prevention, and rights restriction regulations. STAR Services+1
Oversee budgetary, administrative and operational tasks pertinent to assigned programs (e.g., monitoring property/site compliance, training logs, financial oversight). Teal
Qualifications
Meet the designated coordinator qualification standards under Minnesota Statutes 245D.081: e.g.,
Bachelor's degree in Human Services, Education, Health OR
Associate's degree + 2 yrs full-time experience in direct care OR
Diploma + 3 yrs direct care OR
Minimum 50 hrs education + 4 yrs direct care under supervision. MN Revisor's Office+1
Proven experience working with individuals with intellectual/developmental disabilities and/or mental health conditions (preferred supervisory experience). homecareagencymn.com+1

Excellent interpersonal, communication, organizational, and documentation skills.
Ability to interpret and apply regulatory requirements (245D, etc.) into practice.
