SUMMARY:
The Dementia Coordinator will follow dementia-specific, person-centered care plans, conduct resource referrals, and provide light counseling and ongoing case management support to people caring for, and individuals with, dementia. The Dementia Coordinator will contribute to the ongoing development, implementation, and evaluation of the Dementia Navigator program. The Dementia Navigator program services are provided citywide.
Telework Status: This is a hybrid in-person and remote position. The hybrid schedule will be dictated by the business needs of Iona with consideration of the employee's preferences.
RESPONSIBILITIES:
Clinical
Conduct triage of referred cases of older adults with memory loss
Provide telephone consultations and resource referrals (including creating handouts) to DC residents with dementia and their family caregivers.
Conduct home visits and implement dementia-specific person-centered care plans.
Participate in medical visits for clients to assist in supporting clients with understanding doctor's orders
Participate in discharge planning and interdisciplinary team meetings in order to provide dementia-specific expertise.
Maintain client records.
Program
Disseminate information about the Dementia Navigator program to the Senior Service Network and community partners.
Participate in program evaluation including administration of evaluation tools to clients and families and creation of reports on the activities, utilization and outcomes of the Dementia Navigator Program.
Participate in Iona team meetings.
Other duties as assigned.
Qualifications:
Bachelor's degree in social work, with appropriate current licensure in the District of Columbia.
Minimum one year of relevant social work experience required, with knowledge of community resources, and experience with diverse client populations and staff.
Exceptional verbal (written and oral), data management, problem-solving, phone etiquette, customer service, engagement, and active-listening skills.
Excellent interpersonal skills and ability to effectively negotiate conflict situations.
Strong time management skills with ability to prioritize among multiple urgent or emergent client or practice situations.
Strong documentation skills with computer literacy and ability to use electronic records and documentation systems.
Must have a car and valid driver's license.
Spanish speaking is desirable.
