Community Specialist Coach (CCS)
, you'll support individuals with developmental disabilities by helping them engage more fully in their communities and work toward their personal goals. You'll be a trusted advocate, a caring presence, and a vital part of an interdisciplinary care team.
Responsibilities
Implement Individualized Support Plans (ISPs)

Work closely with the care team to help individuals achieve their personal and developmental goals. Track Progress: Use Therapand verbal communication to document daily updates and provide regular feedback on each individual's development.
Be a Positive Role Model
*:
Promote independence, respect, and inclusion through your words and actions. Support Community Engagement
*:
Facilitate outings and activities that build confidence and connection in public settings. Behavior Support:Use positive reinforcement and behavior strategies to encourage success and manage challenges.
Assist with Daily Living Needs
*:
Provide help with hygiene, grooming, light cooking, housekeeping, medication reminders, and other tasks as outlined in the ISP. Provide Transportation:Drive individuals to community activities, appointments, and events using your own insured vehicle.
