CO RESPONDER - CASE MANAGER



JOB SUMMARY:



The Co-responder Program is a collaborative service of the Claratel Behavioral Health and the DeKalb County Police Department to respond to individuals experiencing a behavioral health crisis (mental health and/or substance use issues). The goals of the program are to provide crisis intervention services in the community and to reduce hospitalizations and reduce arrests and incarceration.



The Co-responder Case Manager role helps divert individuals from the criminal justice system and emergency rooms by providing necessary services and reducing repat crisis. The Co-responder Case Manager provides telephone and on-site follow-up to individuals who recently received crisis intervention via education, support, and linkage to community resources. The Case Manager coordinates follow up services and connects individuals and families to resources. The Case Manager conducts homeless outreach, community engagement, and engages with community partners. The Case Manager receives supervision from the program manager.



DUTIES AND RESPONSIBILITIES:



Work closely and collaborate within a team-based model to ensure effective and efficient delivery of care coordination and case management services. Includes telephonic and face-to-face contact. Engages in homeless outreach, community engagement, and engagement with community/law enforcement partners Provides follow-up to individuals who have been hospitalized, have had contact with co-responder unit, and/or who need additional mental health/crisis support Motivate clients to participate in the program by establishing a therapeutic relationship. Maintain a caseload of clients in accordance with department policy, to gain optimal functioning and sustain improvements in health management. Complete comprehensive assessments that collect in-depth information about a client's unique situation and functioning to identify their individual needs. Information gathered should include, but not limited to; medical, psychosocial, and behavioral health. Assist clients in identifying health care goals and coordinating services and providers to meet those goals. Utilize professional judgment and critical thinking to assist the client in overcoming barriers to goal achievement. Modify care plan as necessary. Act as a client advocate and a liaison between internal and external providers, and community resources to seamlessly integrate and coordinate services. Make referrals to community-based organizations and resources that are consistent with the identified needs in the plan of care and/or by the client. Positively contribute to the financial health of the organization by ensuring delivery of billable services and utilizing, monitoring, and collecting data for decision making and program improvements. Maintain a positive, ethical, and professional working relationship with community, government agencies, and other professionals. Participate and contribute to departmental meetings, team huddles, rounds, supervision, and other meetings, as needed, to ensure cohesive department operations. Enhance professional growth and development through participation in specialized training. Performs duties from a recovery oriented, client centered, and trauma informed perspective.

Minimum Qualifications:



Bachelor's degree in social services. At least two years of mental health / substance use disorder experience in assessment, crisis intervention and case management. Must possess basic computer and typing skills. Must be trained in CPI and have current CPR.

Preferred Qualifications:



Psychiatric hospital, crisis stabilization unit, ACT team or co-responder experience.

Master's Degree in Social Work or related field

Excellent interpersonal, problem solving and critical thinking abilities. Must be clinically focused while ensuring best practices. Must be able to remain calm in emergency situations and respond appropriately. Always maintain confidentiality. Professional and effective communication with Police Officer, staff, clients, clients' families/significant others and the public.

WORKING CONDITIONS

Works primarily in the community along with DeKalb County Police and neighboring law enforcement agencies. When not in community, works in a shared office space at Claratel Behavioral Health or Law Enforcement Agency office. Must wear Agency Branded shirt. Potential exposure to highly stressed and emotional situations. Moderate walking, standing, and climbing stairs.
