About the Role

As a Mobile Nurse at Avail Health, you will play an important role in shaping the future of clinical care for nurses and NPs. Joining Avail Health at this early stage offers a unique opportunity to influence the development of best practices in the deployment of technology-enabled clinicians into locations where vulnerable populations reside, while also providing a vital role in the identification and management of chronic illness. You will work closely with a multidisciplinary team, including Geriatric NPs, as well as collaborate with the clinical operations team to deliver comprehensive chronic illness assessments. If we know what ails these patients, we can help them to optimize their health. Your expertise will be instrumental in advancing our mission to transform healthcare.

As part of the West Virginia team, you will be at the forefront of delivering quality care to older adults that is outcome-focused. All administrative details will be taken care of for you - so you can assess, educate and influence the treatment of long-term custodial residents of skilled nursing facilities.

This role begins as a 1099 contracted nurse full-time Monday-Friday.

What:

Support NPs to complete 100 Comprehensive Chronic Disease Clinical Assessments per week

Where:

Skilled Nursing Facilities in West Virginia

When:

Onboarding the week of October 19th. October 27, 2025 - November 26th, 2025

How:

Using Avail Health technology and resources (mobile logistics, patient chart and historical data prepopulated) to perform in-person visits with a virtual NP seeing patients remotely.

Expected Compensation:

Up to $2140 per week with weekly incentive bonuses tied to CDA completion rate and quality of documentation. Additional quality bonus of up to $1000 is offered at the completion of the contract, based on customer bonus payments. Up to $134 daily mileage reimbursement for distances traveled over 30 miles/day ($0.67/mile for up to 200 miles). ~20 hours of remote, asynchronous training and on-the-job training paid at $35/hour

Required Qualifications

Current, active and unrestricted West Virginia or multi-state RN or LPN license Must have your own insured vehicle to travel to and from Skilled Nursing Facilities Experience with geriatric care, treatment of chronic illness Strong commitment to collaboration with other health professionals Comfortable learning new technologies used to perform and document clinical care

Preferred Experience

Basic understanding of Risk Adjustment Factor (RAF) scores, Hierarchical Condition Category (HCC) Codes, capitation payment models and Healthcare Effectiveness Data and Information Set (HEDIS) Measures Experience working within skilled nursing setting Experience with working persons with dementia
