Job Description


\xc2\xb7 As one member of a team of Community Support Specialists (CSS) design, schedule, and implement community-based individual and group employment path outings, learning activities, and social/recreational activities that respond to clients\' interests and their individual service plans.

\xc2\xb7 Provide daily support to a group of individuals based on schedules and events developed by the team of CSS and approved by the Community Services Coordinator and/or the Chief Program Officer (CPO).

\xc2\xb7 Serve as a mentor, support staff, and counselor to assigned participants by developing a clear and concise individualized activity plan for each participant, sharing that plan with other CSS so that activities can be oriented to the participant\'s desires and abilities.

\xc2\xb7 Maintain current case records including progress notes, incident reports, and other documentation required by law or at the request of the Community Services Coordinator or CPO; serve as the primary intervening staff member for assigned participants.

\xc2\xb7 Manage and de-escalate behavior issues for assigned participants including, but not limited to, client anger management, conflict resolution, coping with anxiety, and specific behaviors addressed in the participant\'s ISP plan or formal behavior protocol.

\xc2\xb7 Prepare monthly progress reports and other required documentation on each assigned participant on a timely basis and submit documentation to the Community Services Coordinator.

\xc2\xb7 Communicate participants\' issues to families, group homes, and other service providers as appropriate.

\xc2\xb7 Administer medications and attend to individual medical needs of assigned participants, including first aid as appropriate.

\xc2\xb7 Assist with personal hygiene needs, toileting, and client transfer from wheelchairs when using a restroom or other times when transfer is necessary. Must be able to safely lift and transfer individuals with disabilities as requested.

\xc2\xb7 Safely assist individuals with lunch and snacks assuring those individuals do not consume foods or beverages that will conflict with their medical or dietary plans.

\xc2\xb7 Ensure a safe learning environment at all times; Help individuals have fun and reach their community goals.

Must be able to pass a background check and drug screen.

Essential

\xc2\xb7 High school diploma or equivalent.

\xc2\xb7 Two (2) years of experience instructing/supervising individuals with developmental disabilities OR 4 years of experience in a related field.

\xc2\xb7 Must be able to communicate (verbally and in writing), use a computer for communication of information and data recording, and be able to successfully and tactfully communicate with both large and small groups of clients and their advocates and families.

\xc2\xb7 Must be able to provide individual instruction to assigned individuals and provide a visual presence (leadership) that will hold the attention of individuals in the community environment and entice their active involvement.

\xc2\xb7 Previous experience in planning activity calendars or daily curriculum activities highly desired.

\xc2\xb7 Must possess a valid Oregon or Washington driver\'s license and safe driving record with the ability to drive a 15 passenger van.

\xc2\xb7 Must be able to assist in lifting, repositioning or transferring of clients using wheelchairs. Must occasionally lift 50 pounds or more.
