Job Description

Under the general direction and supervision of the Manager, the Community Resources Specialist plays a proactive role in assisting seniors and adults with disabilities in accessing and receiving social services that will enable them to live independent and healthy lives. Will specialize in providing information and referrals between a wide array of organizations and provide public awareness of long-term service and support options. Will assertively conduct outreach to engage and inform people of all ages, disabilities, and income levels.


ESSENTIAL FUNCTIONS / RESPONSIBILITIES




Community Resource Specialist


Provides one on one assistance for seniors and adults with disabilities to enable them to resolve barriers. Provide information and referrals to clients utilizing a wide array of resources. Services may include, but not limited to, adult day health care, care giver assistance/support, community services, health and wellness, education, emergency preparedness, employment, financial assistance, food/nutrition assistance, housing and shelter, in-home care, legal safety, recreation, senior centers, LGBT programs/services and transportation. Provides up-to-date information and clarification regarding programs such as Medicare, Medicaid, SF CalFresh, and formal supportive and social services. Provides translation assistance, both verbal and written, to clients that cannot speak English, utilizing a language interpreter as necessary. Completes applications, reviews and responds to correspondence, fills out forms, and makes and or confirms appointments. Completes and maintains records for Citizenship and Residency application and keeps up to date with any changes. Conducts clients need assessment. Evaluates client needs, follows-up to make sure the clients' needs are met and document progress notes to ensure quality and effective delivery service for participants. It is vigilant about maintaining accurate and up to date access to resources. Assists in the planning and implementation of a variety of special events and recreational activities for clients. Create long- and short-term plans, including setting targets for milestones and adhering to deadlines. Coordinates communication & outreach events with the ADRC program. Facilitates communication and Collaboration between departments or teams. Represent one organization or group to another, ensuring that interests are aligned. Serve as a point of contact or mediator to members and community. Build relationships, foster understanding and ensure the smooth flow of information with staff and community programs. Attend all regular schedule meeting.

Reporting and Documentation


Completes accurate program and client data collection, data entry, and required programmatic reports on a timely and consistent basis. Maintains confidentiality of all client records, organizes case files documentation both hard files and electronic records. Is compliant with the Health Insurance Portability and Accountability Act of 1996 (HIPPA) privacy and security rules to the extent applicable. Takes all reasonable efforts to implement HIPPA requirements. Comply with all mandated reporting requirements, including Adult Protective Services and HSH. Administers evaluate data collection and participate in program and agency performance management processes. Will aid referrals and completion of N-400, I-912 and I-90 for clients in need of assistance or change of immigration status. Provide monthly reports to Data entry administration and Naturalization partners to ensure delivery is completed. Monthly reporting is required of all outcomes and activities that the program provides. Creates and submits monthly reports of activities and submits monthly metrics in the CARBON system by the 15th of the following month. Enter and submit required data into the SF Get Care monthly. Responsible for submitting Community Services Block Grant time study to HAS/DAAS during the months of February, May, August and November. Develops and delivers ad hoc reports as requested by HAS. Develops and delivers annual summary report of SOGI data collected by the 10th of July each year.

Community Engagement


Actively conducts outreach activities and recruits' clients to ensure full targeted enrollment and ADRC Service Objectives are met. Designs and disseminates outreach materials and informational presentations to generate public awareness of long-term service and support options. Conducts outreach to a wide array of community members and organizations. Develop partnerships and establish communication and rapport between community agencies. Organize and facilitate community-activities in conjunction with the activities Coordinator including but not limited to: Food Pantry, cultural celebrations, large and small group social events, holidays celebrations, skill building workshops, and informational workshops.

Customer Service




Provide exceptional customer service. Always represent the organization in a prudent and professional manner.

Safety




Maintain personal safety, use personal protection equipment (PPE) and follow organization's policies. Report all and any safety issues, accidents, or injuries to your manager as a matter of priority.

Effective Team Member




Ability to interact positively with team members. Ability to facilitate positive change and work as a member of a multi-disciplinary service team. Partner with supervisor when necessary to address department issues or concerns

POSITION QUALIFICATION REQUIREMENTS




Education

: Require a Bachelor 's Degree in Social Work, Human Services, or other related field; or AS in a related field or at least 3 years of social service delivery with elderly and/or adults with disabilities.


Experience

: Must have experience in interacting with seniors and disabled adults, as well as experience in case management, group facilitation and recreational activities. Commitment to social action, social justice, civic engagement, and leadership development.


Requirements:


Current Physical Health Screening (every 2 years thereafter) Fingerprinting Clearance (DOJ) Current Immunizations (TB, Influenza, Covid & Booster) (must be kept current) Current CPR & First Aid Certificate (every 2 years thereafter) Valid State of California Driver's License (current auto insurance and a clean driving record.)

Performance - Ability to handle a variety of tasks simultaneously, prioritize tasks, meet deadlines, able to do presentations and to maintain confident information. Strong analytical, crisis intervention and leadership skills and demonstrate attention to detail and counseling skills. Communication - Strong written and verbal communication skills. Must be a well-organized professional who thrives in a team-oriented environment. Stress Management - Ability to remain tactful under pressure and always present a professional demeanor and communication style with co-workers in a multicultural setting. Experience working with / knowledge of and sensitivity to issues impacting the seniors, disabled, and marginalized populations.

Software: Proficiency in Microsoft Office applications: Outlook, Word, Excel, PowerPoint, Publisher, and Internet Explorer.


Decision Making: Set priorities and standards of performance; Develop plans by which work can be accomplished.


Tools and Equipment: General office equipment, fax, copier, printer, phone, and computer.


Confidential and Sensitive Information: Maintain the highest level of confidentiality when dealing with client records and office performances.


Physical Demands




While performing the duties of this job the employee is required to sit approximately 80% of the time, walk 10% of the time and stand 10% of the time. The employee is frequently required to use hands and fingers to type or handle, feel, and operate objects. The employee is required to occasionally lift and/or move up to 25 pounds. While performing the duties of this job the employee is occasionally required to reach with hands and arms. Must be able to stoop, kneel, bend, crouch, crawl, and work with children on the floor for extended periods of time. The employee is frequently required to speak and listen. Specific vision abilities of this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
