Work Schedule:

Monday - Friday, 8:00am to 5:00pm, 40 hours per week.

Compensation:

$22.28 /hr plus comprehensive benefits package including 100% employer paid pension, generous time off and 14 paid holidays.

General Responsibilities

: Under the supervision of the Access Services Supervisor, this culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Through home visiting and other means of communicating, this individual assists participants in overcoming barriers to health and social services..

Essential Job Functions:

(Reasonable accommodations will be provided, if necessary, for individuals with disabilities who can perform the essential job functions.)Addresses the social determinants of health which effect older adults such as lack of access to good nutrition, economic insecurity, unstable housing, social isolation, depression, discrimination including racial, ethnic, or sexual orientation. Advocates for participants and acts as a liaison between participants and other service providers by working with various community organizations including legal services, domestic abuse service providers, Adult Protective Services (APS), DHHS, community food nutrition organizations and the homeless network. Works in conjunction with the Millage Services Team members with problem solving, completion of applications, and the identification, coordination and delivery of services and support to older adults. Plays an active role in the Crisis Services program by assisting with completing applications for those individuals in need. Member of the TCOA Access services team which includes Information &Assistance and Options Counseling. Participates in all Access team meetings and represents TCOA at identified meetings as needed. Develops and maintains a sound knowledge base of various relief and assistance programs and participates in trainings and webinars as necessary.

8. Assesses the needs of older individuals including mental health and cognitive status.9. Works with persons suspected of being victims of domestic and elder abuse and coordinates with domestic and elder abuse programs.10. Arranges for and provides reassurance and emotional support to older adults, and participants.11. Provides ongoing support and assistance in returning to normalcy following an emergency or crisis situation.12. Determines participant's eligibility for services and completes necessary applications for financial assistance, insurance, and other programs in place to assist individuals and refers participants to community resources and other agencies to meet identified needs.13. Knowledge, SkillsAbilities:

Commitment to the organization's missions and goals. Computer skills sufficient to learn specific software programs. Ability to maintain confidential information regarding all aspects of participant, volunteer, and agency information. Ability to work independently and as part of a team. Ability to actively listen and communicate effectively to establish good relationships with staff, participants, volunteers, and vendors. Demonstrates empathy and sensitivity in meeting participant needs Ability to represent the Agency in a professional manner. Must be able to adjust priorities to meet deadlines in a timely manner. Ability to meet department standards with regard to job knowledge, participant focus, initiative, productivity, communication, teamwork, and attendance.


Minimum Requirements:

Associate's degree in human services or a related field is preferred. A minimum of one-year experience in the field of aging is required. Must possess a current Michigan driver's license in good standing (less than 4 points preferred).


Working Conditions:

The work environment varies. When in the office, safe work practices in regard to office equipment, avoiding trips/falls and fire regulations are required. When traveling to or spending time at residential or medical facilities, exposure to unusual elements such as smoke, unpleasant odors, loud noises, and extreme temperatures increases. Physical mobility is required for sitting, walking, standing, bending, and lifting/holding/carrying objects of up to 20 pounds. Must be able to move around in multiple locations with varying physical environmentsbarriers. Must be able to drive. Ability to enter and access information using a computer. Must be able to communicate effectively with participants, co-workers, and vendors in person and over the telephone. Sensory requirements include exposure to varying temperatures, noise levels, environments, and activities. Mental requirements include the ability to handle varying and intense levels of stress.
