Peachtree Care Health Services is a Medicaid-approved home care agency helping families receive free home care through state programs. We're seeking community connectors - people already in touch with Medicaid-eligible individuals - to join as Referral Partners.
This role is perfect for:
Case Managers, Discharge Planners, Social Workers
Community Influencers (church leaders, tenant association leaders, nonprofit volunteers, etc.)
Anyone who frequently interacts with Medicaid-eligible seniors, adults with disabilities, or families in need
What You Do:
Refer Medicaid clients to PCHS home care services
Share information and connect them to our intake team
Work in your local ZIP code/community
What You Get:
