Employment Type: Full Time | Monday - Friday, 8a-5p
Pay Rate: $30+/ hour (pay is based on requirements and experience)
Location: Kansas City, KS
Sign on Bonus: $2,500


We are seeking a caring and dependable Community RN to support our participants both in the center and in their homes. The Community RN plays a key role in promoting the health and well-being of participants through hands-on nursing care, ongoing assessments, and collaboration with the care team. This position is ideal for a nurse who enjoys building relationships, working in community-based care, and supporting older adults in living safely at home.


Key Responsibilities:


Support Case Managers in implementing care plans and monitoring participant health status
Monitor for changes in condition and report promptly to the provider
Assist in coordinating services, medical appointments, and follow-up care

What we're looking for:


Current RN license in the state of Kansas
Must be comfortable working independently in home settings
Valid driver's license and reliable transportation for community visits
