POSITION SUMMARY: The primary purpose of your job position is to assist the Director of Community Liaison in planning, developing, organizing, implementing, evaluating, and directing social service programs as they relate to the admissions and intake processes and activities of new patients, in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that medically related emotional and social needs of the resident are met/maintained on an individual basis. This includes patient and family education and orientation before, during, and after admission, counseling and problem resolution as needed, and inter-departmental coordination of same. It also includes responsibilities for community outreach and education as outlined below, and as needed at the direction of your supervisor. This is a staff level position with reporting responsibility to the Director Community Liaison.   • Assist the Director of Community Liaison in conducting outreach and education on facility services and availability of patient care services to professional contacts, such as physicians, hospitals, related medical professionals, and discharge planners.
  • Assist the Director of Community Liaison in conducting outreach and education on facility services and availability of patient care services to professional agencies and organizations, such as public health agencies, senior citizen groups, organizations serving disabled persons, and state, county, and local medical societies, or other similar groups and institutions.
  • Assist the Director of Community Liaison in conducting outreach and education on facility services and availability of patient care services to prospective patients, their families or responsible parties, or other related individuals or entities, in multiple locations, including but not limited to hospitals at bedside, assisted living or other congregate living environments, homes, or any other locations in which individuals who may need facility services may be located.
  • In performance of the outreach and education, make known what facilities, services, capacities and availability exist for persons who require this information, relative to the provision of patient care, as well as to serve other purposes such as the exchange and evaluation of medical information for purposes of potential placement.
  • Bedside or other location specific review and evaluation of patients for the purposes of the above, as well as appropriateness for consideration of admission to the facility for purposes of patient care and access to services offered.
  • Provision of written, electronic, or other formatted materials that provide information and education on the services, operations, and other data that contribute to an understanding of the role and function of the facility\xe2\x80\x99s role as a provider of covered health care in the community.
  • Participate in touring and orientation of potential residents/family members.
  • Welcoming and facilitating new admits into the skilled nursing setting, with education and follow up as necessary.
  • Visitation on routine basis with new admits, assisting in the facilitation of necessary services and coordination with all patient care departments as needed.
  • Assist the Director Community Liaison in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility as related to new admissions and the admissions process.
  • Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed and necessary for patient pre and post admission needs.
  • Assist family and resident as necessary in the execution of the required admissions documents.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Participate in discharge planning, development and implementation of social care plans and resident assessment, as needed.
  • Interview residents/families as necessary and in a private setting.
  • Involve the resident/family in planning social service programs when possible.
  • Assist in arranging transportation to other facilities when necessary.
  • Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
  • Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident. Provide direct assistance and support to residents/families with the process of application, submission, and coordination with the relevant programs and their offices.
  • Obtain information concerning the resident\xe2\x80\x99s personal and family problems, past illnesses, etc., by assisting in development of Social Service Data Record.
  • Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
  • Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
  • Maintain contact with the resident\xe2\x80\x99s family, involving them with non-medical progress reports as necessary.
  • Make written and oral reports/recommendations to the Director Community Liaison concerning the operation of the social service department as related to your job responsibilities.
  • Assist in standardizing the methods in which work will be accomplished.
  • Assist in making appointments for the resident/family as requested or appropriate.
  • Work with emotional problems including assisting resident/family with anxieties and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness and death, and the need for institutional and specialized care.
  • Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
  • Evaluate social and family information and assist in determining plan for social treatment.
  • Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
  • Others as deemed necessary and appropriate, or as may be directed by the consultant or Administrator.
QUALIFICATIONS: Certificates and Licenses: \xc2\xb7 Must possess, as a minimum, a high school diploma Experience: \xc2\xb7 Experience in skilled nursing environment, or related health field, as well as experience in public outreach and education. On-the-job training provided for specific tasks. Other Specific Requirements \xc2\xb7 Must provide evidence of being free of tuberculosis infection upon hire and as set forth by the policies of the facility. \xc2\xb7 Must be able to read, write, speak and understand the English language. \xc2\xb7 Supervisory and/or administrative experience in rehabilitative long term care preferred. \xc2\xb7 Knowledge of the Medicare PPS system preferred. \xc2\xb7 Must possess the ability to make independent decision when circumstances warrant such action. \xc2\xb7 Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. \xc2\xb7 Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. \xc2\xb7 Follows written and oral instructions. WORK ENVIRONMENT:
  • Works in resident areas, office areas and throughout the facility.
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Works beyond normal working hours on weekends and holidays, when necessary.
  • Subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Subject to injury from falls, burns from equipment, odors and is exposed to dust, disinfectants, hazardous chemicals, tobacco smoke, and other air contaminants.
  • Subject to hostile and emotionally upset residents, family members, personnel, visitors, or others.
  • Subject to exposure to infectious waste, diseases, and conditions, including Tuberculosis and the AIDS and Hepatitis B viruses.
