Community Liaison / Hospice Marketer - Caring Hands Hospice LLC

Full-Time | Las Vegas & Clark County

Caring Hands Hospice LLC is a compassionate, patient-centered hospice serving patients throughout Clark County. We specialize in caring for individuals with breathing problems and offer a uniquely holistic model including a Physical Therapist for consultation, an on-staff Dietician, and a dedicated, mission-driven clinical team.

Position Overview

The Community Liaison will represent Caring Hands Hospice in the community, promoting our services and building strong, positive relationships with referral partners. This role works closely with our IDG (Interdisciplinary Group) -- including nursing, social work, chaplaincy, medical director, PT, Dietician, and administration -- to support seamless patient care and communication.

Key Responsibilities

Build and maintain relationships with physicians, hospitals, SNFs, ALFs, clinics, case managers & community partners Educate professionals and families about hospice care, especially our expertise in breathing-related conditions Represent Caring Hands Hospice at events, health fairs, and networking opportunities Collaborate closely with our IDG team to support timely admissions and smooth care transitions Track outreach efforts and maintain CRM/outreach logs Identify new growth opportunities across all of Clark County Serve as a compassionate, knowledgeable representative of our mission

Qualifications

Experience in hospice, home health, healthcare outreach, senior services, or similar (1-3 years preferred) Excellent communication and relationship-building skills Self-motivated, positive, and organized Valid driver's license and reliable transportation Understanding of hospice philosophy or willingness to learn Passion for helping families during critical moments
