LifeWays is seeking a passionate Community Health Worker to bridge the gap between health and social services and the community. You'll help individuals access resources, remove barriers to care, and promote wellness while fostering trust and empowerment.

What You'll Do

Provide in-office, in-home, and community-based support to high-risk individuals. Assist Access Services with scheduling, welcome packets, reminder calls, and questionnaires. Conduct Social Determinants of Health (SDOH) screenings and link individuals to resources. Support behavioral health, substance use, and physical health needs through care coordination. Promote recovery and wellness programs, including Health & Wellness groups. Engage in outreach, community education, advocacy, and informal counseling. Maintain communication with individuals and track progress toward health and wellness goals. Attend community meetings and events to represent LifeWays and support individuals. Handle multiple tasks efficiently while maintaining professionalism, confidentiality, and a positive demeanor.

Required Education/Certification/Licensure

Michigan Community Health Worker certification or ability to be certified within 3 months of hire. High school diploma or GED. CPI Training, upon hire. Must possess a valid U.S motor vehicle operator's license, and successfully complete motor vehicle record check.

Preferred Education / Certification/Licensure

Associates degree. Michigan Community Health Worker Certification. * Bilingual (Spanish/English).
