Job Description


Duties:
A. Personal Hygiene
1. Gives baths (partial or complete). Showers as scheduled and when necessary Minimum shower twice weekly.
2. Shampoos hair at least weekly, combs and arranges resident\xe2\x80\x99s hair daily and when necessary.
3. Shaves residents, including removal of facial hair of female residents, trims and cleans fingernails of residents on a weekly basis and when necessary.
4. Provides daily oral hygiene and denture care each shift and when necessary
5. Empties and measures drainage (urine, tubes, vomitus, etc.) as ordered and reports amount, color, and consistency to nurse-in-charge.
6. Gives incontinent and skin care to residents as is necessary.
7. Dresses resident appropriately. Informs nurse of clothing needs and keeps drawers and closets neat and orderly.
8. Encourages the resident to participate in his/her daily activities.
9. Carries out bladder and bowel monitoring and training program as assigned and records results.
10. Checks bowel movements daily and records results on NAAARS every shift.
After three days, of no bowel movement notify the nurse.
11. Assists nurse in dressing changes when necessary.
12. Gives care and support to residents who are terminally ill, isolated, debilitated, or cognitively impaired.
B. Safety and Comfort Measures
1. Assists residents from bed to chair, stretcher and vice versa with use of Mechanical Lift with mandatory two or more person assistance and buddy system assistance
2. Assists ambulatory residents to walk using walker or cane as indicated
3. Participate in resident floor ambulation program
4. Transports residents to clinic, activity programs and other appointments, including but not limited to appointments outside of the facility during working hours.
5. Turns and position residents every 2-3 hours for support and comfort, in the prevention of skin injury and contracture
6. Provides range of motion to residents.
7. Applies support stockings, and splints as directed by the nurse.
8. Assists residents to undress and prepares residents for bed in accordance with residents\xe2\x80\x99 needs and abilities.
9. Answers residents\xe2\x80\x99 call bells and alarms promptly.
10. Positions side rails, as per protocol (Restraint Free environment)
11. Observes and assists restless or cognitively impaired residents as directed
12. Observes any significant change in residents\xe2\x80\x99 behavior or physical condition and informs the nurse. Immediately of noted changes.
13. Respects the rights of residents/patients and families.
C. Vital Signs Procedures
1. Takes and records temperature (orally or rectally as indicated), pulse, respiration and blood pressure into the electronic medical record (EMR)
2. Take and records weights as assigned.
D. Maintenance of Nutrition
1. Prepares residents for meals in a pleasant environment with no distraction.
2. Feeds or assists with feedings as needed.
3. Provides fresh water and nourishment as directed.
4. Encourages fluids as ordered. Measures and records intake and output of residents.(meal consumption sheet) Informs nurse in charge of inadequate intake and /or output.

Skills:

E. Other Related Duties
1. Assists in admissions, transfers, and discharges.
2. Completes clothing check and Communicates property to the nurse on the unit.
3. Ensures residents clothing and supplies are labeled.
4. Participates in clothing inventory and transport during transfers.
5. Makes bed when indicated.
6. Maintains clean and non-cluttered environment in residents\xe2\x80\x99 room and service areas.
7. Observes and reports equipment needing repair.
8. Practices economy in use of utilities and supplies.
9. Documents care on accountability records and forms.
10. Obtains report from nurse-in-charge regarding care delivered (per shift).
11. Gives report to nurse-in-charge regarding care delivered (per shift).
12. Performs other related duties as assigned by charge nurse, supervisor, Associate/Director of Nursing. In an emergency, performs other duties necessary for the care of the residents, or the efficient operation of the facility.
13. Attends in-service programs as schedule for at least 6 six hours every six months, for a total of 12 hours per year.
14. Monitors smoke room as per policy.
15. Monitor residents as assigned by nurse Q15 min and Q30 min.
F. Physical Requirements
1. Ability to walk, stand, sit, bend and stoop for extended periods.
2. Ability to lift, move, push, objects without restrictions.
3. May be required to work beyond assigned working hours; Including weekends and holidays, when necessary

Education, Certifications, Licenses:

G. Minimum Education
1. High School diploma or equivalent (GED)
2. Completion of at least 100 hours from New York State approved residential Health Care Facility nursing assistant training program.
3. Knowledge on the prevention of skin breakdown.
H. Minimum Experience
1. Ability to communicate with residents in a compassionate and gentle manner, ability to understand and carry out verbal and written instructions.
2. Long Term Care experience preferred.
I. Registration/Certification Required:
1. New York Nurse Aide Certification and listed in New York State RHCF Nurse Aide Registry.
