The Clinical Social Worker (CSW) is the initial social work point of contact for newly admitted patients and their family/friends, service providers, etc. CSWs complete initial assessments using the biopsychosocial model and provide services such as case management, facilitating family meetings and case conferences, providing psychoeducation, and facilitating clinical groups and 1:1 interventions. They collaborate closely with rest of the treatment team, patients, and collaterals to formulate treatment and aftercare plans, and they complete all required documentation within the designated timeframes. The listed position may also provide support across treatment teams and psychiatry units when there are coverage gaps within the Social Work team, during periods of increased workload, or when there are other exceptional requests regarding patient care needs on 7L or in PES.
CSWs must collaborate effectively with all team members to ensure coordinated, comprehensive, age specific, and culturally competent clinical care. People in this position work hand-in-hand with their Discharge Social Worker (DSW) counterparts. The DSW is responsible for implementing discharge plans that meet patients' psychosocial needs; they act as a liaison between the patient, the treatment team, natural supports, and community providers regarding the discharge plan. Despite the current separation of CSW and DSW roles, the roles often overlap in their job responsibilities, e.g. mandated reporting, family psychoed, etc. This role distinction may be removed in the future, with each team member providing both clinical and discharge social work services for their own caseload.
The Department of Psychiatric Social Work embraces the Recovery Model of mental healthcare while engaging patients with Motivational Interviewing and other evidenced based practices. Our patients benefit from Trauma Informed Care along with a strong therapeutic program for those with co-occurring disorders.
The Clinical Social Worker reports to the Inpatient Psychiatry Social Work Director.
