About the Department

The Hospice teams provide compassionate support to patients and families during the final stages of illness. Evergreen Hospice serves Linn, Benton, Marion, Polk, Lincoln, and Tillamook counties, offering services in home settings including private residences, skilled nursing facilities, assisted living facilities, and adult foster homes. Support may include nursing visits, pain and symptom management, medical social services, counseling, spiritual care, hospice aides, volunteers, and bereavement care.

Job Summary

The Hospice/Medical Social Worker plays a vital role in supporting patients and families by providing emotional, psychosocial, and resource-based support during end-of-life care. This role requires independent work in the field, professional communication, and strong case management skills.

Responsibilities:

Provide medical social services and emotional support to hospice patients and families Conduct assessments, develop care plans, and coordinate resources Collaborate with interdisciplinary hospice teams including nursing, counseling, and spiritual care Travel to patient homes across assigned counties using personal vehicle Maintain accurate documentation in EPIC EMR Participate in interdisciplinary meetings and care planning

Requirements:

Oregon LMSW or CSWA required Oregon LCSW preferred Healthcare Provider BLS required Valid driver's license, reliable personal vehicle, and required auto insurance Minimum 1 year of experience (hospital or medical setting preferred) Strong communication, assessment, and case management skills

Pay: $54.00 - $57.00 per hour
