The Knox County Health Department (KCHD) and Knox Community Health Center (KCHC)
are committed to promoting and protecting community health through accessible, high-
quality healthcare services. We are seeking a dedicated Clinical Services Nurse (LPN) to
provide essential nursing care and support to individuals and families in our medical clinic
and broader community.
Job Summary:
As the Clinical Services Nurse, you will play a vital role in delivering compassionate,
professional nursing services within the Knox Community Health Center. This position
involves direct patient care, coordination of services, and clinical support to ensure
continuity and quality of care in a patient-centered environment.
Key Responsibilities:
1. Provide professional nursing care in accordance with physician's orders and
recognized clinical procedures.
2. Complete patient assessments, administer injections, perform blood-draws, and
conduct nursing evaluations.
3. Support referrals, diagnostic testing, and follow-up services in a closed-loop
process.
4. Maintain accurate and complete documentation in compliance with billing, clinical,
and grant requirements.
5. Ensure exam rooms are properly stocked and support supply inventory processes.
6. Assist in front desk operations including appointment scheduling and medical
record scanning.
7. Participate in quality improvement and risk management activities.
8. Exhibit courteous, culturally competent, and non-judgmental care to all clients.
9. Operate standard office and clinical equipment efficiently and effectively.
10. Perform other duties as assigned by the Medical Clinic Supervisor or designee.
Minimum Qualifications:
Certification of graduation from an accredited school of nursing.
Current and valid Illinois Licensed Practical Nurse (LPN) license.
Valid driver's license and reliable transportation.
Skills & Abilities:
1. Demonstrated proficiency in nursing techniques, including injections and blood
draws.
2. Excellent communication and documentation skills.
3. Ability to work independently and collaboratively within a clinical team.
4. Strong organizational and prioritization skills with attention to detail.
5. Commitment to continued professional development and education.
6. Comfort working with diverse populations and maintaining client confidentiality.
