The PGx CPP manages medication therapy, within a direct care setting, makes referrals, documents care of assigned patients, . applies knowledge of VA/DOD clinical practice guidelines and Joint Commission standards. The PGx CPP educates healthcare professionals using academic detailing frameworks about PGx principles and appropriate indications and contra-indications for cost-effective PGx testing

Clinical - Direct Patient Care Activities (20-40%):
1) Functions as an advanced practice provider for CMM services to achieve defined outcomes through direct interactions with patients and providers in assigned care area(s)
2) Functions as part of the interdisciplinary team to provide PGx-based CMM

Program Management, Educational, and Administration (60-80%):

1) Advocate for the rational and ethical use of PGx testing as part of routine care
2) Educate healthcare professionals using academic detailing frameworks about PGx principles and appropriate indications and contra-indications for cost-effective PGx testing
3) Provide resources and education that empower patients to make informed healthcare decisions about undergoing PGx testing and understanding their test result
4) Lead diffusion of the PGx pharmacist train the trainer program to offer training to pharmacy leaders, clinical pharmacists, clinical pharmacy practitioners, clinical pharmacy Practitioners, and pharmacy residents
5) Facilitate PGx test results are handled in an ethical manner and that patients are provided access to their genetic data when applicable
6) Provides formal and informal education and in-services to pharmacy, medical, nursing, and other staff as requested
7) Serves as a subject matter expert in PGx for assigned committees, workgroups, or interdisciplinary teams
8) Provides education to patients, their families and caregivers about medications, patient responsibilities, and information on health promotion.
9) Participates in self-development activities to maintain and strengthen professional competence in PGx and precision medicine
10) Willing to serve as a clinical instructor or preceptor for pharmacy students and/or residents
11) Assists in new employee orientation education sessions as needed
12) Participates in the appropriate, effective, and safe use of pharmaceuticals by contributing to criteria development, data collection, quality assurance and drug use evaluation and other quality improvement activities for core practice area(s) medication use and PGx.
13) Assists in the facilitation, evaluation, and implementation of existing, nationally available, PGx programs (e.g., the pharmacogenomics testing for Veterans (PHASER) program) as an integral part of medication therapy if the program is not available locally
14) Assists in the facilitation, evaluation, and implementation, in collaboration with local laboratory services, to offer PGx testing where it is not available (e.g., the HLA gene and corresponding medications: abacavir, allopurinol, carbamazepine, fosphenytoin, oxcarbazepine, phenytoin)
15) Create clinical workflows to facilitate PGx testing for specific patient cohorts (e.g., mental health, patients who will receive a systemic fluoropyrimidine, patients who will receive a drug in which HLA is predictive of SJS)
16) Consider creation of a drug-gene workflow to facilitate PGx testing (e.g., CYP2C19 testing in patients with acute coronary syndrome who have undergone stenting and will receive a P2Y12 inhibitor)

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
  • Education
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure.
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

Reference to the Grade Determinations for GS-12: (In order to qualify for this position you must meet the GS 12 and GS 13 requirements)

Experience, Education, and Licensure. None beyond the basic requirements. (NOTE: See exception to licensure requirement in subparagraph 2c(1) above.)

Please note for board approval the following Knowledge, Skills and Abilities must be demonstrated at the GS-12:

1. Knowledge of professional pharmacy practice.

2. Ability to communicate orally and in writing to both patients and health care staff.

3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.

4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.


GS-13 GRADE:
EXPERIENCE. In addition to the above GS-12 requirements, applicant must have 1 year of experience equivalent to the next lower grade level to meet the GS-13 requirements.

Preferred Experience: In addition to basic Qualification Standard: completion of ASHP-accredited PGY-1 Pharmacy Practice Residency and/or PGY-2 Specialty Pharmacy Residency (in an applicable specialty) or equivalent experience in the major duties for at least 3 years.

Assignment. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.

Facility Program Manager:
Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the KSAs identified below.

a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Preferred experience: Can include completion of a Pharmacy Practice residency; current or previous job as a clinical pharmacist practitioner prescribing under a scope of practice; Pharmacogenomics certification.

Physical Requirements: Must be in overall good health. The work requires standing, walking, moderate lifting and carrying of items, and reaching above shoulders. The work also requires the use of fingers, ability to distinguish basic colors, the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other), and the ability to hear (aid permitted). Must be able to stand up for 4-6 hours a day. Physical requirements for this position will be in accordance with VA Directive and Handbook 5019, HRML No. 05-12-02, Employment of People with Disabilities, Including Under Schedule A, 5 CFR 213.3102(u).
