Jethro Health System is looking for a Clinical Nurse Manager or Director of Nursing to join our team.
The Director of Nursing/Clinical Nurse Manager is an RN who provides oversight of all patient care services and personnel. Ensures quality of patient care in the home. Coordinates care with the interdisciplinary team, patient, family, and referring agency. May also be called Director of Nurses, Supervising Nurse, Director of Clinical Services, Director of Professional Services, or a similar title.

Education:
Graduate from an accredited college
Licensure:
Active state license as a registered nurse (RN).
Experience:
3 years of experience in home health care preferred. Management and leadership experience preferred.
Skills:
Clinical skills as defined as generally accepted standards of practice. Excellent interpersonal skills and the ability to communicate effectively. Demonstrates proven decision-making skills. If required to make visits, proof of current CPR certification.
Transportation:
Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions:
Performs duties in an office environment. If required to make patient visits, may work in patients' homes in various conditions.
Possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
Job Summary:
The
Clinical Manager
is responsible for supervising and coordinating clinical staff, ensuring adherence to
CMS Conditions of Participation, state regulations, and agency policies
. This role involves oversight of patient care services, quality assurance, staff training, and compliance with physician orders. The Clinical Manager plays a critical role in maintaining
high standards of patient care, staff development, and operational efficiency
.
Key Responsibilities: Clinical Supervision & Patient Care Oversight:
Ensure
timely, high-quality, and compliant
delivery of home health services.
Supervise
nurses, therapists, and other clinical staff
, providing guidance and support.
Review
patient assessments, care plans, and physician orders
for accuracy and completeness.
Ensure staff compliance with
OASIS documentation, PDGM requirements, and agency protocols
.
Conduct
case conferences
to review patient progress and update care plans as needed.
Quality Assurance & Compliance:
Implement and oversee the
Quality Assurance Performance Improvement (QAPI) program
.
Conduct
chart audits
to ensure proper documentation and regulatory compliance.
Monitor
patient outcomes, hospitalizations, and infection control data
to identify areas for improvement.
Ensure compliance with
Medicare, Medicaid, and state home health regulations
.
Staff Development & Training:
Provide
clinical education, mentoring, and performance evaluations
for staff.
Conduct
orientation and ongoing training
on documentation, compliance, and best practices.
Address
staff performance issues
, ensuring adherence to clinical policies and procedures.
Operational & Administrative Responsibilities:
Coordinate
patient admissions, discharges, and staff assignments
to maintain workflow efficiency.
Collaborate with
physicians, referral sources, and case managers
to ensure continuity of care.
Assist in the
development and revision of clinical policies and procedures
.
Participate in
state surveys, accreditation reviews, and compliance audits
.
Oasis Knowledge to include hands-on experience with review
Coordinates patient care.
Ensures communication with all physicians involved in a patient's plan of care and other healthcare practitioners.
Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.
Coordinates and integrates care provided by agency staff, including contractual providers.
Coordinates referrals.
Assures that patient needs are continually assessed. Assures reassessment of the patient's needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient's condition, at the physician's request, and after hospital discharge.
Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient and caregiver education and patient, caregiver, and family counseling.
Prepares clinical notes as appropriate.
Assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations.
Participates in activities relevant to professional services including development of qualifications and assignment of Agency personnel.
Participates in Agency-sponsored in-service training.
Recommends resources needed to provide care.
Collaborates on development and implementation of policies and procedures which guide and support the provision of care.
Collaborates with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient care.
Monitors and participates in the QAPI Program and assures appropriate corrective measures are performed.
Promotes compliance with all fiscal intermediary and/or other third-party payors, through education, coaching, and other assistance, as necessary.
Monitors systems and identifies problem areas to the Administrator both verbally and through written reports.
Meets mandatory continuing education requirements of the Agency and licensing board.
Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
Ensures the care offered by the Agency is consistently available to all patients.
