Your analytical ability to plan the transition from where the patient is now to where they need to go, will be invaluable to their well-being. And, as an effective communicator, you\'ll be able to resolve how best to reach different audiences and implement communications based on that understanding.

As the Clinical Case Manager, you will be responsible for the organization and delivery of care management and coordination services for our multicultural inpatient population, based on their clinical needs and available services. In this integral role, you\'ll establish a warm and meaningful relationship with patients and their families, as you provide primary care, including assessment, treatment, care planning, education, advocacy, and counseling.

Primary responsibilities:

  • Coordinate and facilitate patient\'s admissions through discharge including assessment of the patients clinical, psychological, and functional status
  • Collaborate with members of the interdisciplinary team to develop a cost-effective and medically-appropriate individualized plan of care, and monitor services required to achieve quality patient care and resource management
  • Review new admissions to identify patients needing utilization review, discharge planning, and/or case management; and apply clinical expertise and medical standardized criteria to achieve optimal patient outcomes and appropriate reimbursement
  • Identify and document variances affecting the LOS and the discharge planning process and assist in removing internal and external barriers to patient care
  • Make referrals and coordinate follow up services to promote wellness
  • Participate in clinical review initiatives focused on reducing the length of stay and readmissions
To qualify, you must possess:
  • Bachelor\'s degree in Nursing; MSN strongly advantageous
  • Current and valid NYS RN License
  • A minimum of 5 years\' recent acute care experience in Med/Surg or Critical Care areas
  • Certification in Patient Review Instrument (PRI) and Screen
  • Strong knowledge of medical, nursing, and allied health sciences, community resources and case management regulatory requirements
  • Exceptional leadership, motivational, written, and verbal communications skills
  • PC proficiency and familiarity with utilization review, denials, or financial software
