Job Description

JOB SUMMARY:

Oversees case management for clients with Volunteers of America North Louisiana (VOANLA). Case management services include, but are not limited to, assisting clients with facilitating independence, obtaining and sustaining health and wellness, increasing income/resources, and improving social determinates of health.

JOB DUTIES/ESSENTIAL FUNCTIONS:

Supervise, oversee, and support staff responsible for the Choice Neighborhood programs. Recommend to other programs at VOANLA through committee work involving care coordination.



Develop budgets in coordination with staff for respective programs and ensure quality services are provided within the constraints of the budget categories.



Work with staff to ensure program goals are met as defined in operating budget benchmarks, as well as grant requirements and outcomes.



Monitor programs to ensure the agency is in compliance with funding sources and contracts.



Identify, develop and maintain positive community partnerships and relationships.



In conjunction with program staff and the management team, prepare and submit grant



renewals for existing grants as well as grant applications for new opportunities.



Develop, implement and monitor strategic plan for division.



Envision, design, and deliver a model for care coordination for the agency.



Conduct regularly scheduled staff meetings.



Meet training requirements set forth by the agency and funding sources.



Maintain positive relationship with clients, family members and referring agencies.



Work on special projects as assigned.

REQUIRED EDUCATION AND EXPERIENCE:

Bachelor's degree in business administration/management, social work, counseling, or related field.



Three (3) years of management experience including programming, supervision, and budget management.



Current valid driver's license with acceptable driving record.



Automobile liability insurance that meets minimum requirements.
