SUMMARY OF JOB

: This clinical position within Pivotal's Children's Unit provides community-based case management services to severely emotionally disturbed children and adolescents presenting with moderate to severe levels of functional impairment (i.e CAFAS scores of 80-140+ or MichiCANS equivalent). Although this position will take the lead role in providing case management services to the clients, it serves a team approach with other workers in the agency (as well as other agencies) that will often be utilized. This position works closely with Pivotal outpatient staff, Pivotal psychiatric staff, Juvenile and Family Court personnel, Children's Protective Services, school and ISD personnel, and other private and public sector human service providers. This position provides a range of child- and family-focused clinical services and is generally performed on an outreach basis. This position may require the performance of additional duties as assigned. The case manager position allows for a more comprehensive approach in that it treats the child within the context of family and community parameters. It allows for more intensive intervention, and it enhances both child and family functioning, allowing greater ability to mobilize natural supports.

QUALIFICATIONS/EDUCATION:

A bachelor's degree in human services and 1-2 years of experience are required, and 1+ years of job-related experience are preferred. Willing to train. Must meet the requirements of a Child Mental Health Therapist as defined by the Michigan Department of Community Health. Up to 1 year of experience is required. 5+ years of job-related experience preferred.

Specific Job Functions

:

Knowledge of normal/abnormal development

Physical Requirements

:

Requires ability to lift twenty-five pounds.

Prolonged sitting and standing as appropriate.

Stooping, kneeling, crouching, or bending as appropriate.

Able to work in/under high stress/pressure environments with productivity standards and expectations.

Requires coordination and manual dexterity of hands and/or eyes/hand/foot.

Office environment with noise from computers, printers, and telephones.

Use of computer for periods more than 30 minutes at a time.

Possible eyestrain from extended periods of viewing computer screen.

May be exposed to bloodborne pathogens, infectious diseases, and parasites.

Regular travel is required for this position.

Job Duties:



Conducts assessments, which identify customer problems, needs, and strengths Prepares a detailed Individualized Plan of Service for each family served Ensure that person centered plans are completed in a timely manner upon entry to enhanced services and updated at least annually thereafter for primary caseload clients. Conducts treatment planning that is consistent with identified customer needs and follows principles of person-centered planning Ensures timely and accurate documentation of assessments, treatment plans, formal reviews and progress notes according to agency, Medicaid, and CARF standards. Provides ongoing assessment and monitoring of customer needs circumstances, progress, and satisfaction, including monitor progress, or lack thereof, toward treatment goals. Provides assistance in all areas of life to ensure that basic needs are met for clients and their families. Provides collateral treatment with significant others on a time limited basis as needed to achieve state goals Monitor and coordinate services with other agencies, providers, and significant people (e.g. medical professionals, law enforcement, courts, schools, family members, guardians) Coordinates with medical staff, Pivotal's services, psychiatry, respite and emergency services as needed. Coordinates with other human service providers, family physician, probation officers, and substance abuse counselor, school and service providers as needed. Monitors service delivery and progress Provides treatment in a manner that is sensitive to the individual's trauma experience Completes progress and meets productivity. Continually educates the customer about the purpose of service implementation, mutual responsibilities, and potential consequences to noncompliance Continually plans with the customer for discharge from home based services according to customer needs and progress towards stated goals Follows Company's Code of Ethics, Corporate compliance guidelines, standards of business conduct and company policies, (such as attendance, punctuality dress code and work ethic) Protects client's rights by maintaining confidentially and reporting complaints, instances of suspected abuse, neglect or misappropriation of property. Attends and participates in staff development function (such as required training and in-services) Follow principles of Family Voice and Choice (self-determination) Completes annual training log by August 1st Reads and answers e-mail each time he/she is in the office. Listens and responds to voice mail within 24 hours of receiving the messages, unless on leave Other job-related duties as assigned. These duties may include any assignment that provides for the efficient function of the department.

Pay: $16.96 - $27.27 per hour
