: Charge Nurse (LPN/RN)

Department: Nursing

Reports To: Director of Nursing / Assistant Director of Nursing / Administrator

Location: Crestpark Forrest City, LLC

Position Summary:

The Charge Nurse (LPN/RN) is responsible for overseeing and providing direct nursing care to residents in accordance with facility policies, state and federal regulations, and accepted standards of nursing practice. The Charge Nurse supervises Certified Nursing Assistants (CNAs), ensuring the highest level of resident care, safety, and satisfaction.

Essential Duties and Responsibilities:

1. Resident Care and Supervision

Provide and oversee resident care according to physician orders, nursing assessments, and individual care plans.

Administer medications and treatments safely and accurately, following state regulations and facility policies.

Observe, record, and report changes in residents' conditions to the appropriate physician or supervisor.

Supervise, assist, and support CNAs in the delivery of daily resident care.

Ensure proper documentation of resident care and nursing interventions in medical records.

Respond promptly to emergencies, call lights, and resident needs.

Maintain infection control practices and ensure universal precautions are followed.

2. Leadership and Communication

Direct, assign, and evaluate work performance of CNAs and other nursing staff during the assigned shift.

Conduct and participate in shift reports and communicate resident care updates to incoming staff.

Promote teamwork and a positive work environment through clear communication and professional behavior.

Serve as a role model and mentor for nursing and support staff.

Participate in staff education, orientation, and ongoing training programs.

3. Compliance and Quality Assurance

Ensure nursing care is provided in compliance with federal, state, and facility regulations.

Maintain resident confidentiality in compliance with HIPAA.

Assist in investigations of incidents, falls, or changes in resident conditions as directed by the Director of Nursing.

Support facility initiatives for quality assurance and performance improvement (QAPI).

Monitor and report resident safety hazards or equipment malfunctions promptly.

4. Documentation and Reporting

Complete required forms, charts, and reports accurately and timely.

Review and update care plans as needed to reflect current resident needs.

Ensure accurate narcotic counts and medication administration records (MARs).

Maintain and safeguard medical records in accordance with facility policies.

5. Other Responsibilities

Participate in facility meetings, in-services, and continuing education programs.

Maintain professional license and adhere to the Arkansas State Board of Nursing regulations.

Qualifications:

Must hold a current Arkansas Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing.

Must be able to read, write, and communicate effectively in English.

Long-term care or skilled nursing experience preferred.

Strong leadership, organizational, and problem-solving skills.

Ability to work cooperatively with staff, residents, families, and regulatory agencies.

Must possess compassion, patience, and dedication to quality resident care.

Physical Requirements:

Must be able to lift, move, and transfer residents (up to 50 lbs. independently or with assistance).

Frequent standing, walking, bending, and reaching required.

Must be able to handle stressful situations calmly and effectively.

Perform other duties as assigned by the Director of Nursing or Administrator.
