Job Duties:

• The CFTM Facilitator will provide an array of CFTM services as assigned. This role will be key in the development of the Agency\'s CFTM practice region wide.

• Provides an array of Family Engagement services through preparation, planning and facilitating CFTM\'s.

• Identifies/assesses the need for a CFTM and hold a Pre-CFTM meeting with the family to begin preparation for the CFTM.

• Engages families in the family finding process to include Genograms, Ecomaps, Connectedness Charts and CLEAR.

• Documents CFTM\'s accurately in CAPSS.

• Increases community and provider network awareness of DSS family engagement practices such as child and family teaming.

• Will be required to perform Red Cross shelter/emergency response duties as directed.

Agency Requirements:
A Bachelor\'s Degree in Social Work, Social Welfare or Behavioral Science and one (1) year of professional experience in Human Services or Social Service programs.

A Bachelor\'s Degree in any other field and two (2) years of professional experience in Human Services or Social Service programs.

A Master\'s Degree in Social Work, Social Welfare or Behavioral Science. Selected positions may prefer supervisory experience.

A Master\'s Degree in any other field and one (1) year of professional experience in Human Services or Social Sciences.
