Join our dedicated team as a Certified Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. You will work closely with clients, families, and healthcare teams to deliver high-quality home health services that promote comfort, safety, and well-being.

Duties



Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status Provide medication administration following prescribed protocols while maintaining accurate medical documentation Support patients with specialized care needs such as Alzheimer's care, dementia care, memory care, hospice support, and catheter care Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions and medical information securely in compliance with HIPAA regulations Use assistive devices like Hoyer lifts safely to transfer patients while ensuring their comfort and safety (if applicable) Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment for clients

Skills



Strong patient observation skills to identify changes in health or behavior promptly Experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience Knowledge of medical terminology and procedures relevant to home health care settings Ability to work effectively with diverse populations including seniors, individuals with disabilities or developmental disabilities Proficiency in using EMR/EHR systems for accurate medical documentation and record keeping Certification in CPR and first aid to respond swiftly during emergencies Familiarity with hospice care practices and dementia/memory care techniques Excellent communication skills for collaborating with healthcare professionals and family members Ability to operate medical equipment such as Hoyer lifts and monitor vital signs accurately Compassionate caregiving approach combined with strong organizational skills for managing multiple patients' needs
