We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to clients with diverse needs. The ideal candidate will support patients in maintaining their independence, ensuring safety, and enhancing their quality of life through personalized care. This role involves working closely with clients, families, and healthcare teams to deliver comprehensive home health services in accordance with established protocols and regulations. Experience in assisted living, long-term care, or nursing home environments is highly valued, along with proficiency in medical documentation and EMR/EHR systems. The Certified Home Health Aide will play a vital role in supporting patients with conditions such as dementia, Alzheimer's, disabilities, or those requiring hospice or memory care.
Responsibilities
Provide direct patient care including assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility assistance
Monitor and observe patient conditions, including vital signs and overall well-being
Assist with medication administration following prescribed protocols and maintaining accurate medical records
Support patients with Alzheimer's care, dementia care, hospice needs, and memory care requirements
Utilize EMR/EHR systems for documentation of patient interactions, medical records, and progress notes
Operate medical equipment such as Hoyer lifts and catheters safely and effectively
Assist with meal preparation and feeding to ensure proper nutrition
Perform light housekeeping duties including cleaning and laundry to maintain a safe environment
Follow HIPAA guidelines to ensure patient confidentiality and privacy at all times
Collaborate with healthcare professionals to develop and implement individualized care plans
Support patients in community settings or during live-in employment arrangements
Qualifications
Valid certification as a Home Health Aide (HHA) In New Jersey
Experience working with seniors, individuals with disabilities, or in assisted living/nursing home settings preferred
Reminding patient to take medications, and medical documentation practices
Ability to work independently while following established protocols and safety procedures
Compassionate attitude with excellent communication skills and patience for working with diverse populations
Experience using EMR/EHR systems for documentation preferred
Ability to work flexible hours; live-in employment experience is advantageous

Pay: $18.00 - $22.00 per hour
