Responsibilities:
The
Habilitation Associate
will be responsible for providing community based support to individuals based on the needs outlined in the individuals' ISP. Duties includes, but is not limited to:Develop and implement goals for assigned individuals
Prepare daily/weekly activity schedules
Assist and instruct individuals during community based activities as per their goal plans
Collect, maintain, analyze and interpret data on individual's activities, as required
Submit required paperwork within the electronic health record in an accurate and timely manner
Provide transportation to and from activities
Attend meetings, as required and attend 24 hours of in-service training per in-service year
Performs all other duties related to the position as assigned by the supervisor
Demonstrate knowledge regarding fire and emergency evacuation procedures and equipment
Understand that added work or irregular hours may be required due to government agency or funding source requests for special assignments
Qualifications: Must be at least 18 years of age or older
High School Diploma or GED
Valid driver's license Valid automobile insurance Use of personal vehicle Computer skills
Act 34 and FBI Clearances (if applicable)
If non-driving position applicants must have reliable, consistent access to public transport
Minimum Compensation: USD $18.50/Hr. Maximum Compensation: USD $18.50/Hr.
