Seneca Health Services is a Certified Community Behavioral Health Center in wild, wonderful West Virginia. We provide responsible, accessible, and progressive behavioral health services. Youth Crisis Case Managers provide intensive crisis services for individuals who are identified as high risk by school or law enforcement personnel. The goal is to reduce risk of suicide, substance use, or other maladaptive behavior.

QUALIFICATION REQUIREMENTS:

Bachelor's Degree in a Human Service field required. WV Social Work License preferred. Experience in behavioral health and/or substance use disorders preferred.


Valid driver's license and vehicle required.


Must pass a Criminal Background Check and Employment Fitness Determination through WVCARES and a drug screen.


Demonstrated knowledge of basic computer skills required.

PURPOSE:

Provide case management and crisis services for individuals identified as high risk by treatment provider, school or law enforcement personnel, with the goal to reduce risk of suicide, substance use or other maladaptive behavior. Administer behavioral health evaluations, develop and implement individualized treatment plans, provide targeted case management to assure individuals have access to needed services and resources. Provide supportive behavioral health counseling. Essential functions may vary depending on the program/area of assignment.

ESSENTIAL FUNCTIONS:

Collaborate with school systems and local law enforcement to improve student health and well-being by providing education, crisis intervention, and behavioral health services aimed at reducing suicide risk, dating violence, and substance use. Conduct timely crisis follow-ups for students involved with the legal system, assessing individual and family treatment needs and facilitating appropriate referrals. Deliver presentations to students through classrooms or student clubs and provide trauma-informed training to school personnel. Perform comprehensive initial and reassessment evaluations to determine individual strengths, needs, functional levels, and mental health status. Develop, implement, and regularly review individualized treatment plans using a person-centered approach, ensuring services align with assessed needs and life goals. Facilitate linkage and referrals to internal and external services, supports, and resources, and coordinate warm hand-offs to ensure continuity of care. Provide supportive behavioral health counseling to assist clients in achieving treatment goals and managing daily challenges. Advocate for clients to ensure access to integrated, flexible, and appropriate services, and maintain regular follow-up to monitor progress and adjust treatment strategies as needed. Ensure crisis response planning is in place and assist clients in accessing emergency support services when necessary. Serve as a liaison between law enforcement and behavioral health services to divert individuals with low-level, non-violent offenses to appropriate treatment. Participate in interdisciplinary team meetings, treatment planning, physician rounds, and discharge coordination, particularly in residential or inpatient settings. Accurately complete referrals and submit required utilization management documentation to ensure service authorization and continuity. Assure quality service for clients by becoming knowledgeable of and adhering to rules, regulations, and legal requirements and by documenting services provided in accordance with established standards. Secure information by adhering to HIPAA and 42 CFR Part 2 regulations and Seneca privacy policies and keeping client information confidential. Represent Seneca in a positive manner including maintaining a positive attitude and performing duties in a manner in accordance with Seneca's Mission and Code of Conduct and Ethical Practices. Other duties as assigned by supervisor.
