Duties



Provide case management to clients, assisting clients in identifying case management services; as needed, in accordance to best practices. Provide crisis intervention and crisis management support to stabilize clients during emergencies or acute episodes. Accomplish clients care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches. Coordinate services and ensure that clients receive appropriate resources. Collaborate with community agencies, healthcare providers, social service organizations, and government programs to ensure seamless service delivery. Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with organizational policies and confidentiality standards. Support clients in developing life skills such as childcare, employment readiness, and social integration to promote long-term stability.

Experience



Must have high school diploma or GED; an associate degree in human services or a related field is preferred. Must have a QAP (qualified addiction professional) equivalent certification. Strong background in addiction counseling or substance use disorder treatment preferred. Excellent communication skills with the ability to build rapport quickly while maintaining professionalism and transparency. Ability to handle sensitive situations with empathy while adhering to ethical standards and confidentiality requirements. Job Type: Full-time

Pay: From $28.56 per hour

Expected hours: 40 per week
